AABB Issues Association Bulletin Updating Recommendations for HAV Common Source Outbreaks

The AABB Board of Directors has approved recommendations of the AABB Transfusion Transmitted Diseases Committee that address criteria for donor deferral and blood component retrieval in known or suspected common source outbreaks of hepatitis A virus, or HAV, infection. Published in Association Bulletin #13-03, the revisions update previous recommendations for postexposure prophylaxis, or PEP, and management of potentially HAV-infected donors and blood components. The bulletin offers guidance for blood collection facilities about maintaining communication with public health authorities to ensure the institutions remain informed of potential outbreaks and the need for PEP. Additionally, the document provides information related to implementing procedures in response to common source outbreaks, and identifying and managing donors who may have been exposed to HAV. The 2013 recommendations revise the preferred method of HAV PEP from immune globulin to the HAV vaccine and describe actions to be taken after recipients of potentially HAV-infected blood are discovered. AABB now suggests deferral of any donor who may have been exposed to an HAV outbreak regardless of reported vaccination status. The document also includes current HAV incidence rates as well as relevant links to the U.S. Centers for Disease Control and Prevention website.
This bulletin supersedes Association Bulletin #04-08, which was released following an HAV outbreak in Ohio in 2004 that affected 600 people. Release of the revised recommendations concurs with an ongoing investigation into an HAV outbreak — linked to both organic frozen berry blend and organic frozen pomegranate seeds — that has infected more than 150 people in eight states and was reported in this publication on June 7, 2013.

FDA Publishes Guidance on IND Requirements for the Use of Fecal Microbiota

The Food and Drug Administration announced a guidance for industry titled "Enforcement Policy Regarding Investigational New Drug Requirements for Use of Fecal Microbiota for Transplantation to Treat C. difficile Infection Not Responsive to Standard Therapies" in a "Federal Register" notice dated July 18. The document relates to investigational new drug, or IND, requirements for the use of fecal microbiota for transplantation, or FMT, to treat Clostridium difficile. The guidance explains that the FDA will exercise enforcement discretion of the IND requirements when physicians seeking to use FMT to treat C. difficile — that is unresponsive to standard therapies — obtain an adequate letter of informed consent from the patient or the affected individual's legally authorized representative. At a minimum, the informed consent should state that this use of FMT products is investigational and describe the potential risks of the treatment. The guidance does not pertain to other uses of FMT and the policies it contains only are in effect while FDA continues to consider applicable IND requirements. This guidance should be implemented immediately.
The FDA's Center for Biologics Evaluation and Research held a public workshop May 2-3 titled "Fecal Microbiota for Transplantation" and afforded scientists and physicians the opportunity to discuss concerns about the FDA's IND requirements for FMT. The FDA's response to comments made at that workshop was announced in this publication on June 21.

CBER Announces Meeting to Discuss Standards for Cellular Therapies and Regenerative Medicine

FDA's Center for Biologics Evaluation and Research announced in a July 22 "Federal Register" notice a public workshop titled "Synergizing Efforts in Standards Development for Cellular Therapies and Regenerative Medicine Products" that will be held Oct. 7 from 8:30 am - 4:30 pm at the FDA in Silver Spring, Md. The workshop will focus on cellular therapies, or CT, and regenerative medicine, covering topics that include the types of standards and standards organizations that currently exist, standards development as well as their regulatory application, and ways to minimize repetition and maximize collaboration when writing standards for both fields. Additionally, the workshop will allow time for discussion of high-interest subjects associated with standards development for CT and regenerative medicine. Individuals should register to attend by Sept. 23 as seating is limited; streaming of the public workshop webcast also is available.

FDA Posts Notice of Cellular, Tissue and Gene Therapies Advisory Committee Meeting

The FDA posted information about the next Cellular, Tissue and Gene Therapies Advisory Committee meeting that will be held Oct. 22-23 from 8:00 am - 5:30 pm, in Silver Spring, Md. During this meeting, which is open to the public, the committee will discuss topics that include oocyte modification in assisted reproduction for the prevention of transmission of mitochondrial disease or treatment of infertility and updates on guidance documents from the Office of Cellular, Tissue and Gene Therapies; the Center for Biologics Evaluation and Research, or CBER; and the FDA. Additionally, the meeting will cover considerations for the design of early-phase clinical trials of cellular and gene therapy products — a topic that also will be the focus of a guidance that CBER intends to release later this year. Individuals interested in presenting written opinions or information should send in requests by Oct. 15; participants seeking to make oral statements explaining data or arguments should submit applications by Oct. 7.

The July Issue of 'AABB News' Explores Using Patient-Centric Data Metrics to Improve Care

The July issue of "AABB News" explores the role of patient-centered outcomes-based research — which incorporates metrics such as survival, health-related quality of life and severity of symptoms — in helping improve care at the bedside. Several outcomes-based studies are examined in detail within this issue. Additionally, one article highlights a provision of the Affordable Care Act that requires any hospital with more than 50 beds to have a contract with a patient safety organization, such as the AABB Center for Patient Safety. Also included in the issue — mailed this week to members — are pieces written by experts that examine the abbreviated donor history questionnaire and the 2013 American Society for Apheresis Guidelines on the use of therapeutic apheresis in clinical practice.

AABB PBM Webinar in August Addresses Coagulation Management

An AABB patient blood management, or PBM, webinar on coagulation management will be held Aug. 8. This second session in the 2013-14 PBM webinar series will describe the laboratory assessment of hemostatic function and provide guidance in determining the clinical interpretation of coagulation function. Additionally, the webinar will identify the optimal therapeutic options based upon these assessments.
Individuals may purchase all eight webinars of the series as a package to save nearly $90. Non-members may become members for additional savings of more than $150.

Upcoming AABB Webinar Highlights Cleaning and Disinfecting Cellular Therapy Facilities

An AABB webinar to be held Aug. 20 will explore current regulations and recommendations related to cleaning and sanitizing cellular therapy manufacturing sites. The webinar will describe common cleaners and disinfectants and the elements those products should contain. Additionally, the session will explain methods to validate product effectiveness against possible pathogens as well as offer examples of appropriate cleaning practices and procedures for various surfaces.

Reminder: AABB Audioconference Focuses on Factors That Regulate Red Cell Alloimmunization

An audioconference titled "Factors That Regulate Red Cell Alloimmunization: Lessons From Animal Models" will be held July 31 and will describe the function of the immune system relative to red cell alloimmunization. Two researchers — James Zimring, MD, PhD, and Jeanne Hendrickson, MD — will review the factors that affect red cell alloimmunization as well as those related to blood donor biology that may affect patient sensitization to red cells. Additionally, the speakers will explain how this information will affect the practice of human transfusion.
Zimring and Hendrickson are former National Blood Foundation, or NBF, grant recipients; additionally, Zimring serves as chair of the NBF Grants Review Committee and Hendrickson is an NBF Research and Education Trust board member.

Reminder: AABB Offers Preconvention Workshops on Cellular Therapy Products and PBM at the 2013 Annual Meeting & CTTXPO

Members can register for two workshops — focusing on the preservation of cellular therapy products and best practices in patient blood management, or PBM — when signing up for AABB's 2013 Annual Meeting and CTTXPO. A joint workshop that will be conducted by AABB and BioCoR, "The Preservation of Cellular Therapy Products: An In-Depth Collaborative Workshop," will explore the challenge of cryopreserving cellular therapy products. This two-day workshop, held Oct. 10-11, will investigate topics that include the physical and chemical changes of cellular therapy products during freezing, methods of protecting cells and the mechanisms of cell damage during freezing, and fundamentals of quality systems for cellular therapy.
"Patient Blood Management: Patient Care and Outcomes Strategies Workshop," a one-day workshop to be held Oct. 11, will focus on applying the concept of best practices in PBM programs. The workshop speakers will discuss preparing patients for an invasive procedure using the latest PBM strategies and describe research on the use of blood components when treating an upper gastrointestinal bleed. Additionally, the session will include a review of data compiled in the 2011 National Blood Collection and Utilization Survey.
The cellular therapy and PBM workshops require an additional fee.

Region Watch

According to the National Blood Exchange, most types of red blood cells, or RBCs, are plentiful and known needs are being met immediately. Compared with other blood products, AB plasma and O-negative RBCs are in highest demand nationwide and fulfilling requests for them takes nearly one week. The usage of single-bag cryoprecipitate exceeds availability as many blood centers are focusing on producing pooled cryoprecipitate instead of single-bag units. Levels of platelet products are satisfactory and all known needs are being met.

AABB Posts 'Questions and Answers' About Recently Revised WNV Testing Recommendations

AABB posted "Questions and Answers" regarding Association Bulletin #13-02 titled "West Nile Virus Nucleic Acid Testing – Revised Recommendations" that was released June 28, 2013. The Q-and-A document offers responses to questions AABB members have posed to the organization and includes the rationale for the revisions, a summary of changes made to previous criteria, and answers related to reporting presumed/potentially viremic or West Nile virus-reactive donations to the AABB West Nile Virus Biovigilance Network. Association Bulletin #13-02, which was announced in this publication, updates recommendations for nucleic acid testing for West Nile virus that appeared in Association Bulletin #08-03, released April 18, 2008.

HHS Publishes 2011 National Blood Collection and Utilization Survey Report

The U.S. Department of Health and Human Services, or HHS, has released the 2011 National Blood Collection and Utilization Survey report, which provides 2011 data on the collection and transfusion of blood, identifies trends and compares current findings with those of previous survey years. According to the report, the supply of available allogeneic whole blood, or WB, and red blood cell, or RBC, units, after accounting for test discards, was nearly 14.5 million units in 2011, which is a statistically significant decrease of 9.1 percent compared with the supply of allogeneic WB and RBCs in 2008. The 2011 report explains that, over the past several years, blood collectors have made adjustments to correct the oversupply of WB and RBCs that was identified in the 2009 survey report. Additionally, transfusions of WB and RBCs declined significantly by 8.2 percent to 13.8 million units. The blood supply was provided by more than 9 million allogeneic donors — 31 percent of whom were first-time donors and 69 percent of whom had donated previously. A total of 20.9 million components were transfused, including WB, RBCs, platelets, plasma, cryoprecipitate and granulocytes, representing a decrease of 11.6 percent compared with transfusions in 2008. Thirty percent of 2011 hospital respondents indicated that patient blood management now is a formal program in their establishments.
The 2011 HHS report, produced by AABB, is available as a free download on the AABB and HHS websites.

This member benefit offers access to 12 slide set presentations by experts — such as Steven Kleinman, MD, and Darrell Triulzi, MD — on topics that include general information, the pathophysiology of TRALI, and considerations for human leukocyte antibody, or HLA, detection. Specific user experiences with platelet additive solutions and reports from facilities that implemented TRALI risk-reduction measures also are covered within the slide sets.

AABB Issues Summary of Significant Changes to the IRL Standards; New Assessment Tools Released

A summary of significant changes to the eighth edition of the Standards for Immunohematology Reference Laboratories, or IRL Standards, is available on the AABB website. Developed by AABB's IRL Standards Program Unit, or IRL SPU, the summary is intended to assist AABB members with implementation of the new IRL Standards and includes the rationale for the revisions appearing in the eighth edition. Additionally, the document offers the IRL SPU's responses to comments received during the 60-day comment period for the Standards. The eighth edition of IRL Standards becomes effective Oct. 1, 2013, and is available for purchase in the AABB Marketplace.
New IRL assessment tools for facilities and assessors also are available on the AABB website. These resources are free for members and are associated with AABB's revised IRL Standards. The facilities tool can help institutions prepare for AABB assessments, while the tool for assessors provides questions applicable to evaluating a facility's compliance with the eighth edition.

CDC No Longer Recommends Use of Exchange Transfusion to Treat Severe Malaria

The U.S. Centers for Disease Control and Prevention announced, on July 15, that the use of exchange transfusion, or ET — as an adjunct to antimalarial drugs — is no longer recommended to treat severe malaria. ET involves removing a portion of blood from the patient and replacing it with uninfected blood; this process was believed to remove toxins and malaria-infected cells. However, past use of the practice was based upon limited evidence. CDC recently analyzed data from the U.S. National Malaria Surveillance System to determine the efficacy of ET as an adjunct treatment and reviewed the relevant available literature. The agency found that outcomes for patients with severe malaria treated with ET did not differ from outcomes for similarly affected patients treated without ET. The CDC concluded the survival of patients with the condition is not strongly associated with ET treatment.

FDA Seeks Comments on Proposed Information Collection Requirements for HCT/Ps

The Food and Drug Administration requested comments on proposed information collection in a "Federal Register" notice dated July 12. The request relates to the establishment registration and listing, donor eligibility (screening and testing), and current good tissue practice regulatory requirements for human cells, tissues, and cellular and tissue-based products, or HCT/Ps. FDA is asking for comments about whether the collection of information is necessary for the proper performance of FDA's functions; the accuracy of FDA's estimate of the time and effort needed for the collection, including the validity of the agency's methodology and assumptions; ways to enhance the quality, utility and clarity of the collected information; and methods that would reduce the burden of collecting information for the respondents. Summaries of the applicable regulatory requirements are available on the AABB website. Comments should be submitted by Sept. 10.

Transfusion News Video Highlights Transfusion-Transmitted Hepatitis B From Donors With Occult Infection

A video posted on the Transfusion News website features two studies on transfusion-transmitted hepatitis B from donors with occult hepatitis B infection, or OBI — defined in an article in the July issue of "Transfusion" as serologically undetectable hepatitis B virus surface antigen, or HBsAg, in spite of the presence of circulating HBV DNA. In their European-based study, featured in this month's "Transfusion," J. Allain and colleagues found that blood products from OBI donors carry a high risk of HBV transfusion transmission that is dependent on the presence of antibody to HBsAg, or anti-HBs, and viral dose. Additionally, the authors indicated that screening for antibodies to HBV core antigen, or anti-HBc, and HBV nucleic acid testing, or NAT, may be a justifiable safety measure, depending on epidemiology.
In the second study, also in the July issue of "Transfusion," R. Taira et al. evaluated the impact of individual-donation NAT, or ID-NAT, on transfusion-transmitted HBV, or TT-HBV, from donors with OBI in Japan. The authors found that 85 percent of TT-HBV infections that arose from OBI donations would have been prevented by ID-NAT. They further noted that ID-NAT showed that 1.94 percent of donations with low anti-HBc and anti-HBs titers were viremic and that there was no correlation between anti-HBc titers and the frequency of viremia. They concluded that "[t]he elimination of all donations with low anti-HBc and anti-HBs titers would be important to any strategy aimed at preventing OBI-related TT-HBV infections in countries … that have a slightly elevated HBV prevalence in blood donations."
In an editorial in the issue, C. Bianco and R. Dodd state that "[t]he issue that we are still confronting with OBI is that we do not know its actual prevalence among blood donors and its contribution to the residual risk of [TT-HBV] in most regions of the world." They caution that the present studies "contain suggestions that are applicable to the authors' settings, but cannot be comfortably generalized and do not allow construction of clear protocols for donor screening applicable everywhere because of the wide differences in burden of infection and disease in many countries."

The 15th Annual NBF 5K Run for Research/1-Mile Walk will be held Sunday, Oct. 13, within walking distance of the Colorado Convention Center. Runners and walkers can enjoy spirited exercise before attending Annual Meeting sessions. Breakfast and a T-shirt are included in the registration fee. Individuals can sign up for this event by completing the Additional Fee Events section of the Annual Meeting registration form or register separately for the event. At the request of previous years' participants, runners will have the opportunity to raise additional funds for NBF through personalized fundraising — the registration confirmation email will contain information to assist in the creation of a fundraising Web page for the purposes of sharing with colleagues and friends.
NBF's Scientific Research Grants Program provides funding for scientific research in the field of transfusion medicine and cellular therapies and supports educational initiatives to help optimize patient and donor care and safety.
For more information about these events or the grants program, contact Amy Quiggins, NBF manager, at +1.301.215.6551 or via email.

Region Watch

According to the National Blood Exchange, the demand for some frozen products, such as type B and type AB plasma and single-bag cryoprecipitate, exceeds national availability; several days are required to fulfill all requests. Levels of O-negative red blood cells, or RBCs, also are too low to meet all known needs. Supplies of other frozen products and RBCs are robust, and platelet products are plentiful nationwide amid low usage.

DÍA DEL AMIGO EN ARGENTINA, URUGUAY y BRASIL
